I attach mine as follows (all connections use 1/4-inch TS connectors):

CX-3 Left Channel Output -> Art Tube Pre-amp Input
Art Tube Pre-amp Output -> Ventilator Input
Ventilator Outputs (stereo, two TS connections) -> House PA or mixing board.
