It's a Tannerin, formerly known (confusingly) as an "Electro-Theremin". It was invented by Paul Tanner, a session musican, and originally consisted simply of a Heathkit bench oscillator with a lever attached to the frequency knob. Yes, the "keyboard" is just a piece of paper for guidance, and if you think about it, the waveform you hear is quite different to that made by a real Theremin. The story goes that the MU rep at the Beach Boys' recording session needed to take a note of which instrument Tanner was playing, so he just came up with the name "Electro-Theremin".
