Got a great name for it - "OK Chorale"! Sorry about that. But seriously, I think the controls should be:

2-way mode switch - "organ - guitar"
Traditional 6-position rotary control for organ mode
Variable speed and depth controls for guitar mode
Bypass switch, with socket for external footswitch duplicating its function
