I agree too, and I think there is a lot of "snake oil" that goes along with some of those high-end cables. Reasons 1 and 2 are, IMHO essentially non issues if you buy the right type of cable, meaning a cable for stage use as opposed to for permanent installation. Some Belden (for instance) PVC jacket with foil is quite microphonic if you wiggle it around but installed, it does not seem to be an issue.
IMHO, guitar players have to be a bit more picky than us KB players because all 3 reasons you point to are more critical, but to me, even the construction/reliability issue is not as critical because I feel I have more control in my set up to keep my cables out of harms way not to mention they just lie there. I have the low end Pro Co cables from Parts Express of lengths 10, 15 and 25 feet for various configs and have not have a problem yet in the last 5 or 6 years since I bought, but I did add heat shrink on the ends and them of course, proper handling will certainly extend the life of just about everything.
