The relay is indeed the most likely candidate. The contacts get pitted
with age, and eventually can arc enough during switching to weld themselves
shut -- thus, permanent Chorale (slow) speed! But, there is one other
possibility:..

A Leslie 122/142/222 needs about 35-40VDC superimposed on the inputs to
change speeds to Chorale. Removal of this voltage causes Tremolo (fast)
speed to be selected. It's possible that your switching box is broken. Do
you have any way to try the Leslie with another organ or preamp box, just
to be sure?

Having said that, if you DO need to replace the relay, it's a non-standard
mechanical design, by today's standards, and you'll have a hard time
finding it at Digi-Key or Mouser. My suggestion would be to contact a good
supplier of All Things Hammond like BT Productions or Goff Professional,
and see if they can help you out.
